It is a student’s responsibility to ensure their personal details are kept up to date with ACCCO by providing notification and supporting evidnce of any changes, as soon as any changes occur. In the event personal details may change, we require supplementary evidence in some instances in order to update the changes. This form is to guide a student through the process of updating changes to personal details.
Extension Request Form (For Domestic and International Students)
When first enrolled in your course, a Confirmation of Enrolment was issued, listing your commencement date and nominated end date. An end date is the nominated date your ACCCO enrolment time is finished. If the end of your enrolment date is approaching, or your end date has already past and you wish to continue with your studies, you will need to apply for an extension to maintain your enrolment with ACCCO. Failure to provide an extension will result in an automatic cancelling of your enrolment which will result in you being withdrawn from your course. This document is for domestic and international students.
The purpose of this document is to apply for enrolment with ACCCO under a proposed traineeship or apprenticeship contract.
Education and Care Services and/or relevant Government Departments often request that students provide evidence of progression of study (that they are actively working towards a qualification). ACCCO is able to provide this information via a Progression Report. This document is for a student to request a Progression Report. ACCCO will provide up to two Progression Reports free of charge. For subsequent progression reports, a student will incur a fee of $25, as outlined in the Fees and Refunds Policy.
ACCCO will charge $50 inclusive of GST, for each copy supplied for the replacement of lost or destroyed Certificates or Statement ...To apply for the reissuing of a qualification or statement of attainment, you will need to complete this form. Copies of certificates, statements or qualifications will not be re issued until payment has been received.
Request to Defer Study (for Domestic Students)
This form is to request a deferral from studies from the qualification which you are currently enrolled. A deferral is a temporary suspension/postponement of your enrolment. Before proceeding with your request to defer your studies (temporarily suspend your enrolment), ACCCO strongly advises you to seek advice from your employer on whether deferring you study will affect your eligibility for employment, or the current role (position) you may hold within an education and care profession.
Leave of Absence (for International Students)
This form is to request a leave of absence from studies from the qualification which you are currently enrolled. A leave of absence is a temporary leave from your studies where a maximum of 2 weeks may be granted unless otherwise negotiated with ACCCO. ACCCO advises that a leave of absence may affect your student visa. Before proceeding with your request for a leave of absence from your studies, ACCCO strongly advises you to seek advice from your nearest Department of Immigration and Border Protection (DIBP) Office or refer to www.border.gov.au for further information.
Request for Deferment of Studies (for International Students)
This form is to request a deferral from studies from the qualification which you are currently enrolled. A deferral is a temporary suspension/postponement of your enrolment. This form is for International students. ACCCO advises that a deferment of study may affect your student visa. The Student Request to Cancel form can be used by students to formally request their enrolment in a course be cancelled. Students should complete this form and submit to their nominated trainer.
Request for Refund
This document is to be completed when a student/employer is applying for a refund; a remission of debt; or applying to be withdrawn without penalty under extenuating circumstances.
